Seeing your Spanish health records and results online (historia clínica digital)
How to see your appointments, test results and prescriptions online in Spain, using your region's patient portal and the national shared record. For newcomers using the public health system who want to track their care without phoning the health centre.
Terms used
- NIE — The NIE is the identification number Spain gives to foreigners, needed for most official and online procedures.
- Cl@ve — Cl@ve is Spain's online identity system, which lets you log in to public services with a PIN or permanent password.
- Tarjeta sanitaria — The tarjeta sanitaria is your public health card, with a number that identifies you in the regional health service.
- Historia clínica digital del Sistema Nacional de Salud (HCDSNS) — The HCDSNS is the national system that lets regional health services share key clinical documents about a patient.
Key rules
- Regional access: Every region has its own portal, and what it shows differs, so check yours.
- Identification: You need an NIE plus Cl@ve, a digital certificate or a regional login to get in online.
- National record: The shared national record covers only key documents and does not replace your regional record.
- Missing information: If a result or report is not in the portal, ask your health centre.
The short answer
Each region runs its own patient portal or app. Through it you can usually see your appointments and, in many regions, your results. A national system also lets regions share key records, so your data can follow you if you are treated in another region. What you see depends on your region and your health centre.
Find your regional portal
Search for the health service website of your autonomous community and look for the patient portal or app. Examples:
- Cataluña: La Meva Salut
- Madrid: Tarjeta Sanitaria Virtual and Cita Sanitaria Madrid
- Andalucía: ClicSalud+
- Comunidad Valenciana: GVA Salut
- País Vasco: Osakidetza
- Galicia: Sergas
- Castilla y León: Sacyl Conecta
- Castilla-La Mancha: Mi Salud Digital
- Navarra: Salud Navarra (Carpeta de Salud)
Other regions have their own portal too. Ceuta and Melilla are covered by INGESA. Names change often, so confirm the current name on your regional health service website.
What you can do there
Typical functions are:
- Book, change or cancel appointments with your GP, your nurse and sometimes specialists.
- See upcoming and past appointments.
- Read laboratory results, imaging reports and clinical reports.
- See active prescriptions, and request repeat prescriptions.
- See your vaccines.
- Change your GP or health centre, and download your health card or certificates.
- Send messages to your care team.
Some regions show full results and reports. Others show only appointments and prescriptions. If something is missing, ask your health centre.
How you log in
You will need your NIE and a way to prove who you are online. Most portals accept Cl@ve, a digital certificate or the electronic ID card. Some accept a username and password that you get at your health centre or by video identification. Some accept your tarjeta sanitaria number plus a code sent by SMS. Accepted methods vary by region, and the documents list below has the detail.
Without an NIE or an online registration, access is usually limited to phoning or visiting your health centre. Getting your Cl@ve registration done early saves trouble later.
The national shared record
The historia clínica digital del Sistema Nacional de Salud (HCDSNS) lets regional health services exchange key clinical data. That way a doctor in another region can see what matters for your care. It covers a limited set of documents:
- your patient summary
- clinical reports such as discharge, primary care and emergency reports
- test and imaging results
- e-prescriptions
It does not replace your regional record. You can view your data through your regional portal or through the Ministry of Health's citizen folder, using Cl@ve. Which documents appear varies by region.
Spain also exchanges patient summaries and e-prescriptions with other EU countries through a European platform. This is meant for when you need care while travelling in the EU.
Do it in this order
- Get your NIE and set up Cl@ve or a digital certificate.
- Find your regional portal and log in.
- Check what it shows, then ask your health centre about anything missing.
The checklist below walks through these steps.
Documents you need
-
An NIE, the foreigner identification number, which foreign residents need to use the online portals.
-
A Cl@ve registration (PIN or permanent) or a digital certificate such as the FNMT one, or a DNIe electronic ID card. Foreign residents can register in person at an office, by video or with a certificate. This is also how you log in to the national record.
-
A regional username and password, obtained at your health centre or by video identification. Many portals offer this instead of Cl@ve.
-
Your tarjeta sanitaria (health card) number plus a code sent by SMS, which some portals accept.
Checklist
-
Find the portal or app of your autonomous community on its regional health service website, for example La Meva Salut in Cataluña or Salud Responde in Andalucía.
-
Check which documents your portal shows: some show full results and reports, others only appointments and prescriptions.
-
Log in and look at your upcoming and past appointments, and book, change or cancel one.
-
Open your laboratory results, imaging reports and clinical reports, and your active prescriptions.
-
Check that your patient summary, discharge reports and e-prescriptions appear in the national record (HCDSNS) through Carpeta Ciudadana, using Cl@ve.
-
Ask your health centre for anything missing from the portal, since each region decides what it shows.
Frequently asked questions
Can I use the national record if I move to another region?
Yes, the national record (HCDSNS) lets regional health services share key data, so a doctor in another region can treat you. It covers a limited set of documents, such as your patient summary, discharge and emergency reports, test and imaging reports, and e-prescriptions. It does not replace your regional record.
Can I see my Spanish health records online without an NIE?
Usually no, because without an NIE or a registered login your access is mostly limited to phoning or visiting your health centre. With an NIE, you can get Cl@ve or a digital certificate, registered in person or by video.
Can I see my Spanish records if I travel in another EU country?
Yes, Spain exchanges patient summaries and e-prescriptions with other EU countries through a platform called MyHealth@EU. This covers those two document types only. You can see your own data through your regional portal or the Ministry of Health's Carpeta Ciudadana, using Cl@ve.
Will my regional portal show all my test results?
Not always, because the range of features depends on your region and health centre. Some portals show full results and reports, while others show only appointments and prescriptions. If a result is missing, ask your health centre for it.
