Breastfeeding and formula support in Spain

Where to find breastfeeding help in Spain, through the public health system and privately, and what to expect around formula. For new parents and parents-to-be living in Spain.

Key rules

  • First contact: Ask for the matrona appointment at your health centre to reach free public breastfeeding support.
  • Formula cost: You pay for infant formula yourself, except special medical formulas that may be prescribed.
  • Milk banks: Donated breast milk goes to sick or premature babies in hospital, not to families for general purchase.
  • Cow's milk: Cow's milk should not replace formula before 12 months.

Start with the matrona at your health centre

Free breastfeeding help comes through the public health system, and the easiest way in is the matrona. She gives postnatal visits and consultations at your health centre. Ask for an appointment there. Pediatric nurses and pediatricians at the centre also help with feeding questions.

Many health centres run postnatal and breastfeeding workshops or groups. Which ones exist depends on your region and your centre, so ask the matrona what is on offer near you.

In the hospital

Some hospitals hold the Baby-Friendly (IHAN) accreditation. They support skin-to-skin contact and early breastfeeding right after birth. If this matters to you, ask your hospital whether it has the accreditation before you choose where to give birth.

Mother-to-mother groups

Independent groups, such as those linked to La Leche League and local associations, meet in many towns. They are free peer support, separate from the health system. Your matrona or town hall can tell you what runs nearby.

Private lactation support

Private help comes from IBCLC-certified lactation consultants, private matronas and some clinics. Sessions can be at your home, in a clinic or online. You will find most of them in larger cities, but online sessions work from anywhere.

Fees depend on the consultant, the city and the length of the session. Some consultants sell follow-up packages. Ask for the price before you book.

Donated breast milk

Human milk banks are run by public hospitals, for example in Madrid, Barcelona, Valencia and Andalusia. They mainly supply donated milk to premature or sick newborns in hospital neonatal units. Families cannot buy it for general use, and there is no private retail route. If your baby may need donor milk for medical reasons, ask the hospital's neonatal team. Donors are screened mothers who give milk free.

Formula

The public health system does not provide or subsidise infant formula. You buy it at pharmacies and supermarkets.

There is one exception. Special medical formulas, for example for cow's milk protein allergy, may be financed with a prescription. Coverage varies by region, so ask your pediatrician.

Hospitals generally encourage breastfeeding. If you choose formula, staff give it to your baby during the stay. Midwives and pediatricians advise on preparation, quantity and hygiene.

Pediatricians usually recommend starting with a first-stage formula (type 1) and moving to follow-on formula (type 2) later if needed. Cow's milk should not replace formula before 12 months.

The questions below cover the most common doubts.
