Which Spanish residence routes require private health insurance?
Shows which Spanish residence routes oblige you to hold private health insurance and which are covered through Social Security. It is for non-EU nationals choosing or applying for a visa, so you can tell which case is yours.
Key rules
- Non-lucrative visa: You must buy a private policy from an insurer authorised in Spain.
- Employees: Your employer's Social Security registration replaces private insurance.
- Gap before registration: Cover starts only on registration, so nothing covers you before then.
If you come without a Spanish employer, you need a private policy. If an employer registers you with Social Security, you do not. The route you apply under decides which case you are in. Exact wording differs by consulate: take the list for your visa from the website of the consulate that covers your place of residence before you buy.
Routes where you need private insurance
Non-lucrative visa. You must show health insurance from an insurer authorised to operate in Spain. You are not in the public system, so this means a private policy. This is the route retirees and others who live on savings or a pension use.
Digital nomad (international remote worker). You must show health cover. Use a private policy from an authorised Spanish insurer for the visa stage, unless you will be in Spanish Social Security by then.
Student visa. You need insurance valid in Spain, a private policy with an authorised Spanish insurer, on the same conditions as for any other route. School or Seguro Escolar cover applies only in limited cases, so do not count on it.
Investor and entrepreneur visas. Private insurance is required at the visa stage, because you have no Social Security affiliation yet. The golden visa for real-estate investment ended on 3 April 2025.
Routes covered through Social Security
If you come as an employee, you do not need a private policy. This covers employment, highly qualified, EU Blue Card and intra-company transfer routes. Your employer affiliates and registers you with Social Security, which gives you access to the public health system.
Cover starts only when your registration starts, so arrange a bridge policy for the days before.
Family and renewals
A spouse, partner or child joining a resident under family reunification is covered as a beneficiary through the sponsor's Social Security and needs no private policy. A reunified parent is excluded as a beneficiary and needs their own private policy for the whole stay. Family members who apply together with you on a route that needs private cover, such as the non-lucrative visa, need their own policy: see Visa health insurance for a family: one policy or separate ones?.
If you renew a non-lucrative residence, you must prove cover again, so keep your private policy: see Keeping your health insurance compliant at residence renewal.
If you apply from inside Spain
The same conditions apply when you apply or change status from inside Spain. The proof goes to the immigration office (Oficina de Extranjería) instead of a consulate.
If you are an EU, EEA or Swiss citizen, or a UK citizen
- EU, EEA or Swiss citizen: you need no long-stay visa and no visa health insurance. You register in Spain instead.
- Family member of an EU citizen: you follow the EU family-member route, not the visa insurance rules.
- UK citizen with Withdrawal Agreement rights: you follow the Withdrawal Agreement rules and need no visa insurance.
- Other UK citizens: you are treated as non-EU citizens and follow the rules on this page.
If you are self-employed or a freelancer (autónomo)
Your case differs from an employee's. No employer registers you before you travel, so you buy a private policy for the visa stage. Your Social Security affiliation as a self-employed worker only starts after you arrive.
