What covers you before your Spanish health card works
Explains how to get medical care in Spain while you wait for your health card: provisional proof, urgent care, your home-country card in limited cases, or private care. For newcomers who have applied or are about to apply.
Key rules
- Urgent care: Never refused for lack of a card.
- EHIC or GHIC: Temporary stays only.
- Provisional proof: Ask for it when you register; the format depends on the region.
- Private costs: Money paid for private care while you wait is generally not refunded.
What to rely on while you wait
Until your Spanish health card works, you rely on one of four things: provisional proof of registration, urgent-care rights, a card from another country (only in some cases), or private care.
Provisional proof of registration
Many regional health services issue a temporary certificate once you are registered. It is a printed paper or a digital card. It lets you book appointments at your health centre before the plastic card arrives.
This is common in regions such as Madrid, Andalucía, Cataluña and Valencia. The form and availability differ by region and even by health centre. Ask at your health centre or regional health service when you register.
Urgent care without a card
Emergency departments cannot refuse urgent care because you have no card. Everyone in Spain has the right to:
- urgent care for serious illness or accident, until medical discharge
- care during pregnancy, childbirth and afterwards
Children under 18 get full care.
Do not skip emergency care out of fear of the bill.
Your card from another country
An EHIC or GHIC only covers temporary stays. Once Spain is your home, you need Spanish public cover or private insurance instead.
If your home country still pays for your care, as for posted workers, students insured at home and S1 pensioners, its card can still cover temporary needs: details.
Private care
Private insurance is the safest way to cover routine care in the gap. If you pay for private care yourself, expect not to get the money back once your card arrives. Public reimbursement exists only in narrow cases, such as a proven emergency where you could not use public services in time. You would claim from your regional health service with documents.
Keep every invoice and medical report either way.
If you have a chronic condition, a disability or a dependent adult
To continue treatment and prescriptions during the wait, take private insurance that covers pre-existing conditions. Or bring a prescription and a medical report. Public prescriptions need the working card. Continuing treatment is covered in Continuing physiotherapy and chronic pain care in Spain.
If you work remotely for a foreign employer or are not working
Your work does not give you Spanish public cover automatically, so plan on private insurance.
- Remote worker for a foreign employer: On digital nomad routes you have no public care, so you need private cover from arrival.
- Not working: You need recognition from the INSS or private insurance; after one year of residence the convenio especial is the third option. If your permit depends on private insurance, you have no public healthcare, so your policy must cover you from the day you arrive.
Documents you need
-
Provisional proof of registration (a printed certificate or digital card), issued by your regional health service or health centre, to use the system before the plastic card arrives
-
Supporting documents for a reimbursement claim to your regional health service, only if you paid privately in a proven emergency where public services could not be used in time
Warning: using someone else's card or claiming cover you lack
- Using another person's card: Using another person's health card, or claiming cover you do not have, can get your card cancelled and leave you owing the cost of the care. Only use a card issued in your own name.
- Care without entitlement can be billed: If you receive care without valid entitlement, the health service can bill you, or the insurer or third party that must pay. Emergency care is always given, but its cost can still be billed. Unpaid bills can be pursued through administrative enforcement. Ask the clinic for the price before non-urgent care. If you get a bill you think is wrong, appeal it with the regional health service that sent it.
